The short version

  • Both are compounded, prescription-only sublingual combinations that dissolve under the tongue.1,2
  • Rugiet Ready: sildenafil + tadalafil + apomorphine, from $7.29 a dose.1,3
  • Quattro: sildenafil + tadalafil + vardenafil + apomorphine, from $115 for 10 doses.2
  • Refunds: Rugiet refunds only before your medication is ordered; Quattro has no money-back guarantee and charges only after a clinician approves.4

Rugiet and Quattro are built on the same idea: instead of choosing one ED medication, combine several in a single dose that dissolves under the tongue. That makes them more alike than any other pair in this category, so the details decide it.

What is in each

IngredientWhat it doesRugiet ReadyQuattro
SildenafilPDE5 inhibitor; peak about 1 hour, ~4-hour half-life5✓✓
TadalafilPDE5 inhibitor; effect up to 36 hours6✓✓
VardenafilPDE5 inhibitor; absorbed well in dissolving form7—✓
ApomorphineDopamine agonist acting on arousal in the brain8✓✓

Rugiet does not list its doses on its product page and says dosing is personalized.1 Quattro's doses are also set by the prescribing clinician. The practical difference is vardenafil: Quattro includes all three PDE5 inhibitors, Rugiet two.

Price

Rugiet lists Ready from $7.29 a dose and is HSA and FSA eligible.3 Quattro starts at $115 for a 10-dose month, which works out to $11.50 a dose, with a larger pack available at a lower per-dose price.2 Both include the clinician consultation in the price.

Speed and duration

Both companies say their product works in about 15 minutes and lasts up to 36 hours; Rugiet bases its figure on a survey of 114 of its patients.1,2 Neither claim comes from a head-to-head clinical trial, and no such trial exists. What the published research does show is that sublingual and dissolving forms of PDE5 inhibitors can be absorbed somewhat faster than tablets,9,10 that tadalafil's effect lasts up to 36 hours,6 and that sublingual apomorphine produced a median time to erection of about 19 minutes in a clinical trial.8

Refunds and guarantees

Rugiet's terms say it does not offer refunds or returns except before the medication has been ordered, or if its medical provider disqualifies you.4 Quattro does not offer a money-back guarantee either; its card is charged only once a clinician approves the prescription, so a medical disqualification costs nothing.

The consult

Both start with an online questionnaire reviewed by a licensed clinician. Rugiet adds a video visit only when required and includes unlimited follow-ups.1 Quattro is prescribed by a state-licensed clinician after an asynchronous review and filled by a state-licensed 503A compounding pharmacy.2

Which to choose

If the lowest per-dose price is the priority, Rugiet starts lower. If you want all three PDE5 inhibitors in one dose, Quattro is the more complete combination. Either way, both are compounded drugs that are not FDA-approved, and a clinician needs to confirm a combination is safe for you.11

Considering Quattro?

Quattro is a compounded sublingual dose combining sildenafil, tadalafil, vardenafil and apomorphine, prescribed only after a licensed clinician reviews your health history. It starts at $115 for a 10-dose month.

Important safety information. PDE5 inhibitors (sildenafil, tadalafil, vardenafil) must not be taken with nitrates in any form, including nitroglycerin, and need caution with alpha-blockers.5 The most common side effects are headache and flushing.5 An erection lasting more than four hours needs emergency care.6 Apomorphine commonly causes nausea, especially at higher doses.8 Only a licensed clinician who knows your health history and medications should decide whether any of these are safe for you.
