Hims and Ro are the two largest direct-to-consumer ED services in the U.S. Both run an online intake reviewed by a licensed provider, both ship free, and both sell FDA-approved generic sildenafil and tadalafil alongside compounded products of their own. Here is how they compare on the things that matter, using each company's own website as of September 30, 2026.

Pricing
Ro publishes a per-dose price list, which makes it easy to compare: generic sildenafil runs $4 to $10 a dose depending on strength, and generic daily tadalafil is $8 a dose.1 Hims prices by the month, starting at $22 for generic sildenafil and $24 for generic tadalafil.2 If you use only a few doses a month, Ro's per-dose pricing is likely to come out lower; if you want a daily option, compare the monthly totals directly.
Formats and combinations
Hims leans into formats: berry sildenafil chews, lemon tadalafil chews, and wintergreen "hard mints" combining sildenafil and tadalafil. Hims labels these as compounded and not FDA-approved.2 Ro's answer is Sparks, a sublingual dose of sildenafil 55 mg and tadalafil 22 mg at $12 a dose, also compounded, which Ro says takes effect in about 15 minutes on average.3
Both combinations pair sildenafil with tadalafil. Neither adds vardenafil or apomorphine, the other two compounds used in some combination products. For background on how each medication differs, see our guide to sildenafil, tadalafil and vardenafil.
The consult
Both services begin with an online questionnaire reviewed by a licensed provider; neither requires a video call in most cases. Ro describes its online visit as taking about 20 minutes, with a provider review in about a day.1 Research on online ED pharmacies has found that the quality of counseling varies widely between platforms, so read the safety information whichever you choose.6
Refunds
Neither company offers a money-back guarantee on prescription medication. Ro states that all sales of prescription products are final, except for a filling error.4 Hims says it cannot guarantee a refund once the pharmacy has processed an order, though its providers can adjust treatment if something is not working.5
If you want a sublingual combination
If the reason you are comparing is that a single medication has not been enough, the more relevant comparison may be between combination products. Quattro by Vea Health is a sublingual dose that combines all three PDE5 inhibitors, sildenafil, tadalafil and vardenafil, with apomorphine.7 Like Sparks and the Hims mints, it is compounded and requires a clinician's approval.8
Considering Quattro?
Quattro is a compounded sublingual dose combining sildenafil, tadalafil, vardenafil and apomorphine, prescribed only after a licensed clinician reviews your health history. It starts at $115 for a 10-dose month.
